Important Instructions for parents
Make checks payable to: M.I.T. or MADRONA INTEGRATED TEAM.
· DO NOT allow students to sell door to door without adult supervision.
· Collect money when taking orders. DO NOT collect sales tax.
· On or before OCTOBER 13, return the envelope with the TOP
TWO copies of order form and PAYMENT inside, to the box in the
Main office. DO NOT return the Innisbrook catalog or the third order form.
Keep these for your records until your product arrives.
